Care Planning
OnePlan allows us to create customised care plans to meet the specific needs of each individual. This includes:
A personalised brief of the client - their likes and dislikes, hobbies, pets, any interesting and relevant stories, etc
What tasks they require at each visit
Their medication regime
Medical details
Schedule the visit times as agreed and assign the best suited carers
Monitor Carers as they complete their tasks and safely move from one client to the next
Family Login
A key feature of the Oneplan system is a section where family members can log in securely and see details of what happened in the care visit. The system can be accessed globally 24/7.
Family access allows the family to see:
The personal information relating to the client
Care Schedule – when the care times are allocated and which Carer will attend
what care tasks have been completed for each visit
Any incidents that have been raised
Recent medication administered
Carer notes about the visit e.g. clients wellbeing, moods, and observations
